BIG what’s there not to get? The tooth fairy mob is making that dentist take teeth out of people that don’t need them out for payment for something. Maybe protection money. Oh and saying first in a thread = internet loser who’s never kissed anyone.
Okay, BIG. He’s basically saying that the “tooth fairies” are actually an organized mafia who collect teeth (for god knows what), and for reasons unknown, the dentist has gotten involved and indebted to them. The only way he can keep his skin is to give them what they want. Teeth. Hence ripping out poor little Jimmy’s “extra” teeth.
The lucrative field of dentistry is just one big conspiracy, to put it shortly.
It’s just a play on the situations people that get involved with cartels and the like find themselves in.

When a child loses a tooth, the child is told tooth fairies will take the tooth and leave behind money, grant a wish or some other incentive. Usually, the parent encourages the child to put the tooth under the pillow, in a tooth box or in somewhere else.
Also worth noting, in a lot of western civilisation, the lower jaws of people are getting smaller, to the extent that a significant proportion of mouths can no longer sustain the usual quantity of teeth. This is something dentists try to find out by doing a scan of the teeth at an early age, before all the teeth have come through, so that clashing teeth can be removed.
So, this story mocks the two: a known kid’s story and the crippled evolution of mankind: claiming the former is no story and the later is not true, or to put it simply, it is all one big cover-up.

Personally, I think he put a whole new spin on how to look at the joke for people who had no knowledge of that information before his comment regarding the shrinking of the jaw.
Or he could be over-complicating a decent joke to some other people, depending on how you look at it ultimately.
